Interesting post - but I don't see why focusing on accountability leads to low trust, and why individual performance doesn't matter to be in a high trust organization.
I want my team to be accountable for their changes, and good individual performance leads to high trust in the person doesn't it? Focusing on team performance all the time means some people do most of the work and remain largely uncredited and underappreciated.
